Also, how is defence assigned in the case of Instant Kill? For example, if I defend 30 and get hit by a 30 instant hit and a 30 regular attack, will I get killed?

As per the rules of Rumble, you decide how you take damage received from attacks, so, assuming you decided to not take damage from the Instant Kill (I will, in fact, assume this as the game goes along simply because nothing else really makes sense), you would not die. In a word, no.
